Go to the City of Narak
Go to the west gate and talk to the Sergeant Guardsman, ask Sergeant walls.
Then go talk to to the Sultan of Narak, ask Sultan writ. He will give you a writ allowing access into the walls.
With the writ in your inventory go back to the Sergeant Guardsman and talk about the walls again, he will let you into the walls to start your murdering spree.
Bring some cure disease and cure poisons with you.
